Implement the scene table with defines (#1128)
* Implement scene table with a single define

* Format z_scene_table.c

* Apply review suggestions

* Reorder arguments in DEFINE_SCENE
* Use ROM_INFO for the scene table
* Change "Debug only" to "Debug-only"

* Apply more review suggestions

* Use NULL for 'none' rom addresses
* Remove scene declarations from segment_symbols.h
